The Science of Nebulisation
Nebulisers generate aerosol droplets using different mechanisms. Two common designs are compressor or jet nebulisers and vibrating-mesh nebulisers:

Compressor Nebulisers: These use compressed air to generate an aerosol from liquid held in the nebuliser chamber. They are well-established and reliable devices, although many models are larger and noisier than portable mesh nebulisers.

Sterile isotonic saline contains approximately 0.9% sodium chloride, giving it an osmotic concentration broadly compatible with body fluids. When nebulised, it forms an inhalable aerosol that can interact with mucosal surfaces throughout the respiratory tract. Where those droplets ultimately deposit depends on several factors, including aerosol particle size, breathing pattern, airflow, airway anatomy and the characteristics of the individual nebuliser. It is therefore more accurate to think of saline nebulisation as a method of delivering moisture-containing aerosol through the respiratory tract rather than as a guarantee that saline will coat a particular part of the vocal folds.

What Is a Steam Inhaler or Vocal Steamer?
A steam inhaler for singers provides a different form of humidification. Vocal steaming involves breathing warm, humid air generated from heated water. This may be achieved with a purpose-designed electrical steam inhaler or, more traditionally, by using warm water in a bowl. Because hot water and steam can cause burns, temperature and safe handling are important.
The Mechanism of Warm Steam
Unlike a nebuliser, which produces an aerosol of liquid droplets, a steamer increases the moisture content of inhaled air through heated water and water vapour. As the warm humid air travels through the upper respiratory tract, cooling and condensation occur along the airway.
For many singers, the principal attraction of steaming is the sensation of warmth and moisture in the nose, mouth and throat. It may provide temporary subjective relief from sensations of dryness or congestion and can form a relaxing part of a pre- or post-performance routine. From Early Atomisers to the Modern Nebuliser
Although portable mesh nebulisers may seem like distinctly modern devices, the idea of delivering moisture and medicinal substances to the respiratory tract as a fine spray has a history stretching back more than 150 years. During the nineteenth century, physicians and inventors experimented with increasingly sophisticated methods of transforming liquids into droplets small enough to be carried in moving air and inhaled.
An important development came in 1858, when the French physician Jean Sales-Girons introduced a portable apparatus known as the pulvérisateur. A hand-operated pump drew liquid from a reservoir and forced it through a nozzle, producing a fine spray. Although primitive compared with today’s electrically powered nebulisers, the device represented an important step towards portable therapeutic aerosol delivery.
Further progress followed rapidly. In 1862, the German physician Bergson developed an air-jet atomiser known as the Hydrokonium. Instead of relying on the same pumping mechanism, it used rapidly moving air to draw liquid upwards through a tube and break it into droplets. This exploited pressure and airflow principles associated with Daniel Bernoulli and Giovanni Battista Venturi. Variations of this fundamental idea would later become central to jet nebuliser technology.
Steam provided another means of generating the pressure required for atomisation. During the 1860s, Dr Emil Siegle developed a steam-powered inhalation apparatus in which steam from a small boiler powered the atomising process. The Siegle-type steam atomiser became influential, and related designs spread through Europe and elsewhere during the later nineteenth century. By the late 1860s, medical literature was already describing numerous atomisers and nebulising devices designed to deliver inhaled preparations.
Inhalation and Spray Technology in the 1880s
The historical illustrations shown in this guide come from the German illustrated periodical Die Gartenlaube, published in Leipzig in 1884. They provide a particularly interesting snapshot of how sophisticated spray-generating apparatus had become by the late nineteenth century.
The first illustration shows hand-pressure atomisers (Handdruck-Zerstäuber) operated with rubber bulbs. The 1884 article discusses designs associated with Benjamin Ward Richardson and Joseph Lister, in which compressing the bulbs generated airflow through the apparatus to produce a continuous spray.

The second illustration shows several steam-powered atomisers (Dampf-Zerstäuber): (a) a small Waldenburg atomiser with safety valves; (b) a portable steam atomiser; (c) a large carbolic-acid steam spray used principally for surgical purposes; and (d) Heinrici’s decorative “spray fountain” for dispersing carbolic solution into room air.

In these devices, hot steam passing across the opening of a tube created a pressure difference that drew liquid upwards from a reservoir. The liquid was then carried into the moving stream and dispersed as a fine spray. The underlying concept—using a rapidly moving gas to draw up and atomise a liquid—is recognisably related to principles subsequently employed in pneumatic aerosol generators.
There is, however, an important historical distinction. Not every apparatus in these illustrations was intended specifically for direct respiratory inhalation. The 1884 article was concerned substantially with spraying solutions into rooms and with contemporary ideas about disinfection and air treatment. Some of the illustrated devices therefore belong to the wider history of atomisation and spray technology rather than being direct equivalents of today’s medical nebuliser.
This distinction makes the illustrations particularly useful. They show that the modern nebuliser did not appear as a single isolated invention. It emerged from a much broader nineteenth-century experimentation with inhalers, atomisers, steam-powered sprays, compressed-air systems and therapeutic aerosols.
Over the following decades, these technologies became progressively smaller, safer and more controllable. Modern compressor nebulisers replaced hand pumps and steam boilers with electrically powered airflow, while contemporary vibrating-mesh nebulisers use a microscopic perforated membrane to generate aerosol without the bulky pneumatic machinery of earlier systems. The technology has changed dramatically, but the fundamental objective remains recognisable: turning a liquid into an inhalable aerosol in a controlled and practical way.
The Important Difference Between Nebulisers and Steamers

- Nebulisers typically deliver an aerosol at or near room temperature. Their purpose is aerosol delivery rather than warming the upper airway.
3. Liquid Used
- Steamers should be filled only with the type of water specified by the manufacturer. Salts, essential oils and other additives should not be introduced unless the device is specifically designed and approved for them.
- Nebulisers used for saline inhalation should be used with sterile saline intended for inhalation and in accordance with the manufacturer’s instructions. Prescription medication should only be nebulised when directed by an appropriate healthcare professional.
4. Portability and Convenience
- Modern mesh nebulisers can be very quiet and compact, with many battery-powered models suitable for travel, rehearsal rooms and backstage use.
- Electrical steamers are generally larger and require water to be heated before use. Traditional bowl steaming also involves hot water and therefore carries a burn and spill risk, making a purpose-designed device a more controlled option for many users.
Can Steam Actually Reach the Vocal Folds?
A common debate within the vocal pedagogy community is whether steam actually reaches the vocal folds. The answer requires some anatomical and physical nuance.
When you swallow, the airway is temporarily protected so that food and liquid are directed towards the oesophagus rather than the trachea. Drinking water therefore does not normally wash directly over the vocal folds. Instead, regular fluid intake contributes to systemic hydration as water is absorbed and distributed throughout the body.
During breathing, air travels through the larynx and between the vocal folds. When warm, humid air is inhaled, it therefore passes through the same airway. However, steam inhalation is not equivalent to applying liquid water directly to the vocal folds. Temperature, humidity and condensation change continuously as the inhaled air travels through the respiratory tract.
For singers, steaming is better understood as a form of warm humidification. It can increase the humidity of inhaled air and may provide temporary relief from sensations of dryness or irritation in the upper respiratory tract. Many performers also find the warmth subjectively soothing.
Nebulisation works differently. A nebuliser creates an aerosol of microscopic liquid droplets that can travel with inhaled air through the respiratory tract. Depending on particle size, breathing pattern, airflow and airway anatomy, droplets may deposit at different points in the upper and lower respiratory tract, including the laryngeal region. This is why nebulised saline has attracted particular interest in voice care, although it should not be described as guaranteeing that saline coats the vocal folds directly.
